To apply for accommodations with Student Accessibility and Inclusive Learning Services:
- Please submit an registration form.
- When the application opens, please fill it out as much as you can. Some information will auto populate with your ASURITE sign in information. Anything with a star (*) next to it is required.
- When you are finished, click Submit at the bottom of the page. The form will time out if left for an extended period of time and you may need to fill it out again.
After submitting your application, you will receive a confirmation email to your ASU email address. Within the confirmation email, you will have infomration on the next steps in the process including setting up an intake appointment to discuss your disability and to determine what accommodations would be reasonable.
