If you are applying for assistance from the
Department of Economic Security, you can submit the DES form by mail, fax or in person to
Financial Aid and Scholarship Services to complete. Please be sure to include your 10-digit ASU ID number on the form.
Please note: You must sign the
authorization to release information section before it will be completed.
The financial aid staff will verify the dollar amount of Title IV funding that you are receiving. If a fax number for DES is provided, our office will fax the completed form to DES. This process can take two to three business days for completion. If no fax number for DES is provided, or if you prefer to pick it up in person, please revisit the front counter where the form was submitted in two to three business days.
