How do I apply for the Arizona Education and Training Voucher?

To apply, you must complete the following forms:

• The Foster Care to Success Student Portal offers one application to apply for both the Arizona Foster Youth Award and the Arizona Education and Training Voucher. The application opens each July 1 for those entering ASU that August (fall semester). 
• The Financial Aid Release Form must be printed, filled out and submitted to ASU Financial Aid and Scholarship Services each semester. Financial aid will complete the form with your estimated cost of attendance and a list of all your other awards (i.e., loans, grants and scholarships). The completed form will be faxed to ETV and show your estimated need.
• The ETV Student Participation Agreement explains your responsibilities as a program participant and must be signed and faxed or mailed to ETV before funding is disbursed.
