How Do I Opt out of E-Signature After I Have Sent an Email Request to My Parent Through Verification Documents Request?

If you and your parent would rather submit a signed form after you have submitted an email request to your parents, please follow the steps below.  
  1. Log back in to My ASU and click on Verification Documents Request.  
  2. Under the Household Form bar, click Cancel Request.  
  3. When a pop up box appears, click Yes.  
  4. Then, click Fill Out.  
  5. Go to the Review and Sign section.  
  6. At the bottom of the page, slide the button to opt out of e-sign.  
  7. Then, click the download button to print.  
  8. Once you and your parents both sign the form, either scan it or take a picture with your phone. Then, upload the saved scan or picture.
