CISA Medical or compassionate withdrawals are intended for students facing extraordinary circumstances—such as serious illness, injury, or significant personal situations—that prevent them from completing their coursework. Each request is evaluated individually, and approval is not guaranteed. Approved withdrawals result in a grade of "W" on the transcript.
Types of Withdrawals
- Medical Withdrawal: Applicable when a serious physical or mental health condition hinders class attendance.
- Compassionate Withdrawal: Applicable for significant personal situations (e.g., death of a family member, birth, divorce, eviction) that impede academic progress.
Required Documentation
- Withdrawal Request Form: Complete the Request for Documented Medical/Compassionate Withdrawal Form for each semester you're requesting withdrawal.
- Personal Statement: Explain the circumstances leading to the withdrawal request and the date(s) the circumstance occurred. For partial withdrawals, detail why specific courses are affected.
- Supporting Documentation:
- Date of illness onset
- Dates of medical care
- General nature of the condition and its impact on coursework
- Anticipated return date
- Last date attended classes
- Medical Withdrawal: A letter from your healthcare provider on official letterhead, including:
- Compassionate Withdrawal: Documentation varies based on the situation (e.g., death certificate, legal notices)., but must include dates.

How to Apply
- Complete the Request for Documented Medical/Compassionate Withdrawal Form.
- Gather supporting documentation from a licensed medical or mental health professional.
- Email the completed form and documentation to cisamcw@asu.edu.
Review Process
- Upon receiving all necessary documentation, you'll get an email confirmation.
- The CISA MCW Committee reviews requests on a first-come, first-served basis. Processing typically take 10 - 14 days but may be longer during peak times.
- You'll be notified of the decision via email.
