How to request medical records at ASU Health Services?

ASU Health Services and Counseling Services maintains records on an Electronic Medical Record (EMR) system in accordance with applicable State and Federal laws. Your original medical record is the property of ASU, but the information in it belongs to you.

Your Health Services and Counseling Services medical records are separate records, and each requires a specific authorization request. Before receiving copies of your records or authorizing records to be disclosed, you can submit a request via your Health Portal or complete an authorization form. If providing the form, you will need a picture ID. To avoid a copying fee, request that your records be sent to your health portal account or sent directly from our facility to another facility for continuing care. Processing times for non-medical emergencies can take up to 10 business days.

How to upload your request form:

Log in to your Health Portal:  Login
Locate the messages tab
Select “I want to communicate with Health Medical Records”
Upload your attachment
