ASU Health Services provides vaccination and travel health information for students preparing to travel outside the United States. A travel appointment should be scheduled as far in advance as possible (at least 4-6 weeks), since some vaccines are given as a series over a six-month period.
To schedule a travel appointment with Health Services, please follow the steps below.
- Complete the ASU Health Services International Travel Questionnaire form. The form must be completed and submitted prior to making an appointment at ASU Health Services.
- Submit your form by fax to 480-907-3040 or deliver in-person at any of our four campus locations to the ASU Health Services reception desk.
- Following submission of the International Travel Questionnaire form, you can schedule your appointment online through your Patient Portal or in-person at any of our four campus locations.
Please note: For study-abroad programs that last several months, you will want to make sure that you have completed all of your routine health check-ups, like seeing your dentist or other specialists.
