How to schedule a travel vaccine appointment?

ASU Health Services provides vaccination and travel health information for students preparing to travel outside the United States. A travel appointment should be scheduled as far in advance as possible (at least 4-6 weeks), since some vaccines are given as a series over a six-month period.

To schedule a travel appointment with Health Services, please follow the steps below.
  1. Complete the ASU Health Services International Travel Questionnaire form. The form must be completed and submitted prior to making an appointment at ASU Health Services.
  2. Submit your form by fax to 480-907-3040 or deliver in-person at any of our four campus locations to the ASU Health Services reception desk.
  3. Following submission of the International Travel Questionnaire form, you can schedule your appointment online through your Patient Portal or in-person at any of our four campus locations.
Please note: For study-abroad programs that last several months, you will want to make sure that you have completed all of your routine health check-ups, like seeing your dentist or other specialists.
