Use this form to report suspected identity theft or fraud involving an application to or enrollment at Arizona State University (ASU). ASU will use this report to investigate whether an account exists and to flag it as potentially fraudulent. If federal loans were taken out in your name, that will be addressed in step 3 listed on the above page.
