What is Identity Verification and Statement of Educational Purpose?

Information
The U.S. Department of Education notifies ASU Financial Aid and Scholarship Services when you must confirm your identity and certify that the federal student aid that you receive will only be used for education purposes. 

To fulfill this requirement, complete the Identity Verification & Statement of Educational Purpose form in person at any Financial Aid and Scholarship Services location. If you are unable to appear in person, you may complete the form in the presence of a notary with an attached copy of one form of valid, unexpired government-issued photo identification.* 

Acceptable forms of unexpired identification are:
  • driver's license
  • non-driver's identification card
  • other state-issued identification card
  • U.S. passport
Please note the following:
  • If the notary attaches an All-Purpose Certificate of Acknowledgement, it must include what type of ID was used in verifying the your identity.
  • School IDs, bank cards and non-photo identification are unacceptable.
  • This form cannot be faxed.
  • This requirement is not applicable to scholarships or employee tuition benefits.
  • This requirement does not determine your eligibility to register for classes.
*Government-issued ID is one issued by the U.S. government, any of the 50 states, the District of Columbia, Puerto Rico, a federally recognized American Indian and Alaska Native Tribe, American Samoa, Guam, the U.S. Virgin Islands, the Commonwealth of the Northern Mariana Islands, the Republic of the Marshall Islands, the Federated States of Micronesia, or the Republic of Palau.
