Please refer to your ASU email received for details regarding the necessary supplemental information to attach. Examples include but are not limited to:
- paid receipts (not billing statements)
- marriage certificate
- divorce decree or legal separation paperwork
- W-2
- last pay stub with year-to-date earnings
- tax return transcript
- unemployment benefit documentation
- severance pay documentation
- signed letter from a childcare provider
- signed personal letter
- death certificate
- survivor benefits documentation (i.e., Social Security survivor benefits, life insurance distributions, annuities, etc.)
