The process for applying for a meal plan exemption is as follows:
- Download the designated Meal Plan Exemption form.
- Complete the first page of the form with the required information.
- If your exemption request is related to a medical condition, then the second page must be completed by the appropriate medical professional. If your exemption request is related to a religious dietary observance, then the second page must be completed by your spiritual advisor.
- Submit the completed documents to University Housing by one of the following methods:

- Email: housing@asu.edu (please include your name and topic in the subject)
- In Person: Student Services Building (Monday - Friday, 8 a.m. - 5 p.m. MST)
- Notification of exemption request decision will be sent via email to your ASU email account
Please note: You may be asked to provide additional documentation or meet with the Sun Devil Dining Nutritionist after submitting your request to determine whether or not an exemption is warranted.
